Healthcare Closer to Home – BEN PCT
These pilot projects develop personalised care for patients in their own homes, shortening the time they have to spend in hospital and preventing them from being admitted unnecessarily. The pilot projects began in December 2009 across east and north Birmingham, and will reshape the following services:
- Enhanced Supported Discharge following hospital admission for fractures
- Acute Community-based Care Service for long term conditions such as chronic obstructive pulmonary disease (COPD)
- The Family Liaison Service
- The Herceptin at Home service for breast cancer treatment
Among the groundbreaking new services being offered are a rapid response team, new care pathways and a dedicated care bureau. The care bureau will provide a 24 hour telephone nurse triage facility, enabling patients with these services to call in when they need to receive medical advice.
